When an order ships in three boxes, the carrier charges for three boxes. If the quote was for one, the difference comes out of margin. Hazmat regulations make this worse.
Dim weight ignored
Carriers bill the greater of actual weight or dim weight. A light item in a large box is billed as heavy. Most systems miss this on every dimensionally significant shipment.
Single-carrier rating
USPS Priority is often 40-60% cheaper than UPS Ground on small parcels. LTL is often dramatically cheaper above 150 pounds. Single-carrier systems leave it on the table.
